Please bring the following original documents along with the completed enrollment packet:

1. To verify address (bring one of the following):

A current utility bill issued to the parent/guardian (i.e. Department of Water and Power, or Gas Company or Income Tax, or Lease/Mortgage Agreement.  We do not accept telephone bills or driver's license as proof of residence.

2. To verify age (bring one of the following):

Original Birth Certificate, or Dully attested Baptismal Certificate, or Passport, or an Affidavit, or Vital Statistics Record of Birthdate.

3. To verify immunization:
Immunization Record/ Vaccination Card, Mantoux T.B. Test and reading  (at least 1 year from the date the student will be enrolled).  Polio (total of 4 vaccines), DTP (total of 5 vaccines, MMR (total of 2 vaccines), Hepatitis B (total of 3 vaccines), Varicella (Chicken Pox) (total 1 vaccine).

Dear Parents,

In order to save you time, please make sure that all information requested is completed.

1. Your son/daughter must be 5 years old by December 2, 2011 to qualify for enrollment.

2. Your son/daughter must have the required immunizations including the Mantoux (TB) test with result before he/she can be admitted and it must be current (at least 1 year from the date that he/she will be enrolled).

3.  To assure proper placement of your son or daughter, please provide us with a copy of his or her last two years Report Card and the last two years of his or her State Standardized Test results.

4.  If your son or daughter has an I.E.P., please provide us with a current copy of it.
